Lethal injection became the dominant execution method in the modern United States because it was presented as a more controlled and less visibly violent alternative to electrocution, gas, hanging and firing squad. In practice, the method has produced a long record of litigation, protocol changes, drug-supply problems and execution failures.
How the method is structured
U.S. jurisdictions do not use one universal lethal-injection protocol. Some protocols rely on a single large dose of a barbiturate or sedative. Others use multiple drugs, traditionally combining a sedative or anesthetic with a paralytic agent and a drug intended to stop the heart. The exact drugs and sequence have changed repeatedly as manufacturers, pharmacies and regulators restricted access to execution drugs.
Why IV access matters
A recurring problem is establishing and maintaining intravenous access. Many of the best-known failed execution attempts involved teams that could not place a functioning line before the execution warrant expired or officials called off the procedure. Other cases involved infiltration, where drugs entered surrounding tissue instead of circulating as intended.
One-drug and multi-drug protocols
The phrase “lethal injection” therefore describes a family of state procedures rather than one fixed technique. Some states moved toward single-drug pentobarbital protocols after controversies surrounding older three-drug combinations. Others have changed drugs when supplies became unavailable. These changes have repeatedly generated litigation over whether a protocol creates an unconstitutional risk of severe pain.
Legal and operational controversy
The U.S. Supreme Court has upheld lethal injection against broad constitutional challenges, but litigation often focuses on the details of a particular state's protocol, the drugs available, the qualifications of the execution team and the prisoner's medical condition. Drug secrecy laws and confidential supplier arrangements have also become a major part of the modern debate.
